As early as 1989, Slash brought up the possibility of recording an entire cover album. Four years later, it's a done deal with this fifth album. The Spaghetti Incident? debuted at number 4 on the Billboard charts, selling about 190,000 copies in its first week of release.

Miscellaneous
- Many of the tracks were recorded with original Guns N' Roses guitarist Izzy Stradlin during the Use Your Illusion sessions and then were later re-recorded by Gilby Clarke.
- The album title is an inside joke referring to a food fight between Axl Rose and Steven Adler involving spaghetti.
- On this album Slash picked out to cover "I Don't Care About You" by Fear, "Buick Makane" by T-Rex and the "Hair Of The Dog" by Nazareth.
- In the end of "Hair Of The Dog" one can hear the refrain of The Beatles song "Day Tripper" being played.
- On "Ain't It Fun" Axl is sharing lead vocals with Michael Monroe.
- On very initial pressings, "Look At Your Game Girl" was at track 99, with tracks 13-98 all being short silent samples.
- In recent re-issues of The Spaghetti Incident?, "Look At Your Game Girl" has been added as a separate, 13th track.
- During studio sessions the band recorded a cover of "A Beer And A Cigarette" from Hanoi Rocks but the song never came out.
